Program Year 2017 (March 1, 2017 through February 28, 2018)

Lucas County offers three medical plan options for eligible employees and their eligible dependents for the 2017/2018 program year: Paramount Patient Center Medical Home Plan (PCMH), a 90/10 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay. Employees in the PCMH plan must meet with their participating physician, complete a physical exam and enter into a "Partnership Agreement" with their physician. Paramount Health Care (PHC), a 75/25 HMO plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay; and the Lucas County Health Benefit Plan through the Frontpath Network, a 70/30 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay.
