You may order copies of certificate by phone, mail, in person or online with a credit card.

By Phone: *NEW IN 2012* You can now order your Birth/Death Certificates by phone using a major credit card (Discover, MasterCard or Visa). Call 419-213-4100 Option 6 to place your order. In most cases, certificates will be mailed the same or next business day.

In Person: Visit the Vital Statistics Office at the Toledo-Lucas County Health Department. In most cases your certificate will be processed and issued in about 15 minutes.

By Mail: Birth Certificate or Death Certificate form. Mail the completed forms along with payment for each certificate and a self-addressed stamped envelope to Vital Statistics at the Toledo-Lucas County Health Department at 635 N. Erie Street, Toledo, OH 43604. Upon receipt of your request, your certificates will be mailed back to you within 2 business days. Please allow 3 additional business days for the return mail. When requesting by mail please include the following information: Name on record, Date of birth, County of birth, Mother's maiden name, Father's name.


Online: For your convenience, you can process online requests through VitalChek Network, Inc. - an independent company that the Toledo-Lucas County Health Department has arranged with to provide you this service.
VitalChek may be reached through their web site. An additional fee of $7.00 (plus shipping) is charged by VitalChek for this service. All major credit cards are accepted, including American Express, Discover, MasterCard or Visa.
