Comparison of Benefits Chart
Program Year 2017 (March 1, 2017 through February 28, 2018)
Lucas County offers three medical plan options for eligible employees and their eligible dependents for the 2017/2018 program year: Paramount Patient Center Medical Home Plan (PCMH), a 90/10 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay. Employees
in the PCMH plan must meet with their participating physician, complete a physical exam and enter into a "Partnership Agreement" with their physician. Paramount Health Care (PHC), a 75/25 HMO plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay; and the Lucas County Health Benefit Plan through the Frontpath Network, a 70/30 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay.
