Program Year 2013 (March 1, 2013 through February 28, 2014)

Lucas County offers three medical plan options for eligible employees and their eligible dependents for the 2013/2014 program year: The Lucas County Health Benefit Plan through the Healthspan (formerly Physicans Health Collaborative) Network, a 90/10 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay.  Paramount Health Care (PHC), a 75/25 HMO plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay; and the Lucas County Health Benefit Plan through the Frontpath Network, a 70/30 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay.
