Birth Certificates
The Toledo Lucas County Health Department can now provide certified copies of Birth Certificates for any person who was born in the State of Ohio.  In the past you would have to obtain the certificate from the county of your birth.  Statewide Issuance gives you the ability to obtain your Birth Certificate without the inconvenience of traveling to your birth county or applying by mail.  You can now easily obtain the certificate right here in Lucas County.

Death Certificates
Death Certificate for an individual who passed away in Lucas County may be obtained from the Toledo-Lucas County Health Department.

Certificate Fees
The fee for certified copies of birth or death certificates is $25 per copy. Methods of Payment include cash, check, credit card or money order.  Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates.
