Comparison of Benefits Chart

Program Year 2020 (March 1, 2020 through February 28, 2021)

DUE TO COVID-19:  No employee will incur a $500 health plan deductible for not completing an annual physical by December 31, 2020.

DUE TO COVID-19:  No employee will moved from the Paramount PCMH Plan to the Paramount HMO plan for not meeting the PCMH plan requirements by December 31, 2020.

Patient Centered Medical Home Plan Requirements:

  1. Employee must complete their annual physical by December 31, 2020 (Including routine blood tests as ordered by their physician).
  2. Employee must complete Partnership Agreement with their physician and submit to Paramount by December 31, 2020.
  3. When recommended as part of your Partnership Agreement, employee must participate in mandatory educational nutrition counseling sessions at least twice during the plan year.
  4. Employee must refrain from using tobacco or vape products or complete a smoking cessation program during the plan year.
  5. Any employee who does not meet all of the above requirements, will not be eligible for this plan the following year.

Lucas County offers three medical plan options for eligible employees and their eligible dependents for the 2020/2021 program year: Paramount Patient Centered Medical Home Plan (PCMH), a 90/10 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay. Employees in the PCMH plan must meet the requirements outlined above.  Paramount Health Care (PHC), a 75/25 HMO plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay; and the Lucas County Health Plan through the Frontpath Network, a 70/30 plan with a $10 PCP office visit co-pay and a $15 specialist office visit co-pay.  The benefit levels through each of these three plans are not the same. Employees who are enrolled in any one of the medical plans, will have a payroll deduction of $25.00 per single contract per month and $50.00 per family contract per month.  The payroll deductions will be taken pre-tax.  This means the actual amount taken out of your paycheck will be $11.54 per pay for a single plan and $23.08 per pay for a family plan.
