All employees, spouses, and dependent children age 13 and older, who are eligible and/or enrolled in Lucas County Benefits, are eligible and encouraged to participate.
Any expenditure incurred that is not included in a Lucas County Employee Wellness Contract is specifically excluded from reimbursement under this program. Reimbursement is not provided unless the specific terms of the contract are fulfilled. Reimbursement for any personal property is excluded under this plan.
All former employees are ineligible for this program. All employees eligible for Lucas County Health Benefits, but not actively at work because they have exercised their rights under the Family Medical Leave Act are eligible to participate in this plan.
